PHIERStations

• PHIERStations  – community organizations and safety net programs that use the PHIERS suite of services to participate in coordinated interventions that provide referrals, track outcomes, reward participation and pay for performance.  

Regional PHIERStations can collaborate and compete with local ACOs by providing less costly care and access to resources that can yield better outcomes for more people at a fraction of projected costs, qualifying PHIERStations for performance-based grants and contracts nationwide. 

Performance-based grants will be awarded to facilities, programs, job seekers and employers  that participate in our PHIER-powered interventions.
