PHIERworks

  • PHIERworks is a sustainable job training and placement program that will use existing tools, training, resources, and support to strengthen the safety net and tax base needed to fund programs important to our future.  We will coordinate our efforts with the U.S. Climate Mayors and federal/state health and labor departments to promote rapid adoption by safety net programs across the nation. The entire program was designed to augment and complete what is being left undone by the President’s approach to healthcare reform and Opportunity Zones.

We will train job seekers and front-line health and social workers to use Myhealthevet, the Veterans Administration’s existing web-based/mobile personal health record designed to address the needs of veterans and Myhealthepeeps, the public version of Myhealthevet, to coordinate care and transportation for veterans and duals:  

    • They will update and/or share the patient’s health record (with patient-informed consent) before and after each encounter [with a provider and/or facility] to keep everyone up to date with the latest patient information
    • They will assist in the intake and assessment of the patient’s health and social services needs
    • Advocate on behalf of patients, while providing the tools, training, and support needed to help them (and family caregivers) better advocate for themselves

Every job we create will be 100% self-sustaining, paid for by savings resulting from keeping participating veterans/duals out of the ED.
